What Is an Apicoectomy?

An apicoectomy is a minor outpatient surgical procedure in which the root tip (apex) of an infected tooth is removed along with the surrounding infected tissue. The end of the root is then sealed with a biocompatible material to prevent reinfection. This approach allows treatment of the problem area without disturbing the coronal portion of the tooth or its existing crown.

When Is Microsurgery Recommended?

Endodontic microsurgery is indicated when root canal retreatment through the tooth is not feasible or has not resolved the infection, when the anatomy of the root tip is contributing to persistent pathology, when a root canal instrument has fractured in the lower portion of the root, when there is a perforation at or near the root apex, or when a biopsy of periapical tissue is needed for diagnosis.

What to Expect During the Procedure

The procedure is performed under local anesthesia. Dr. Aziz makes a small incision in the gum tissue near the affected root, gently retracts it to expose the underlying bone, and removes the infected tissue and the root tip. A small preparation is made in the end of the root, which is then filled with a bioceramic material. The incision is closed with sutures, which are typically removed within a week.

Most patients experience mild soreness and swelling for two to three days following surgery. Over-the-counter pain medications are usually sufficient for managing discomfort.

Modern endodontic microsurgery — performed with magnification, microsurgical instruments, and advanced sealing materials — has reported success rates of 85–95% in current literature, significantly better than older surgical techniques.
